SSN and Name Changes

Social Security Number Changes:

  1. Print and sign the below SSN / Name Change Form.
  2. You must also provide a copy of your Social Security card with your SSN / Name Change Form. The copy of the Social Security card must reflect the new number and/or name. 
  3. Deliver your SSN / Name Change Form and copy of Social Security card* either in person, by mail, email, or fax. Delivery in person or by mail is recommended as the security of information via email and fax is not guaranteed.

*Please note, Social Security cards are required for name changes to verify and remain in compliance with the IRS records.

Name Change Requests:

  1. Print and sign the below SSN / Name Change Form.
  2. You may request to change either your full legal name (Last, First and Middle), or your preferred unofficial first name. Preferred name changes only apply to your First name, are not an official name change, and will not display on official documents.
  3. You must also provide a copy of your Social Security card with your SSN / Name Change Form. The copy of the Social Security card must reflect your new number and/or name. 
  4. If you do not have a Social Security card, you must provide a copy of your passport, birth certificate, or signed legal court document (a marriage license will not be accepted).
  5. To make a change to a suffix, complete the form and provide a copy of your SSN.
  6. To correct an issue with spacing, capitalization, or punctuation, please fill out the name change form and provide a copy of your state ID.
  7. Deliver your SSN / Name Change Form and copy of Social Security card either in person, by mail, email, or fax. Delivery in person or by mail is recommended as the security of information via email and fax is not guaranteed.

Please note:

  • Social Security cards are required for name changes to verify and remain in compliance with the IRS records.
  • If you intend to apply for graduation this term, don’t delay submitting the application, the name change can still be applied to your official graduation record. Deadlines do apply for your name to appear in the commencement program.

Change of Address (Former Students Only):

Current students: Permanent Mailing, Emergency, and W2 addresses can be changed online through MySam. If you require assistance changing your W2 address, please contact the Payroll Office.

  1. Print and sign the below SSN / Name Change Form.
  2. Provide the old address and the address you would like it changed to.
  3. You must provide a copy of your state ID to verify your identity.
  4. Email the SSN/Name Change form and a copy of your state ID to graduationteam@shsu.edu.
